How to Write a Sworn Statement

Use plain paper. Top left: your full name and address, plus optional email and phone. Top right: recipient's name, address, email, and phone. Below: location and date, e.g., "Paris, January 12, 2015."
Left-align: "Subject: Sworn Statement."
Start the body: "Dear Madam/Sir, I, the undersigned [your surname and first name], residing at [your address], swear that [state the fact or situation]. I acknowledge that false certification carries criminal penalties and attest to the truth of the above facts."
Close with: "Made to serve and assert what is right."
